I see people walking in the streets, smiling, eating and buying things. Of those people we see every day, some have diseases, some have pain, some feel physically uncomfortable, and some are diagnosed with cancer but, you and I don’t know that. Those people come to the hospital seeking treatment, a cure; hoping that doctors will ease their pain. Many times they come out but other times they don’t. Disease, illness, sickness, anything that gives us pain is the result of something. Whatever the cause, the present circumstance is what it is which in my opinion, can be deviated by our own thoughts and attitudes. Yes, hope is the start, and we all start right; but only patience can take anything to where there is less pain and more happiness. I learned through this program that pain goes away when we stop judging/hating (this applies to the person and to the world). Therefore, my antidote to any disease is to love, with all the imperfections, your body, yourself, them people, and them places in the world :) We all need one another and if circumstances ask (illness), let's all come together like the family we are, and express the love we've lacked.

The last week was significant… I shared many special moments with my host family whom I became very close to. I will never forget the great experience I had during my stance at their house… they are the best Bolivian family without a doubt. On the other hand, during hospital rotations, I got into the advanced chagas consultations; cardiopathies that frequently occur after the chagas parasite is not dormant in the body anymore; usually seen in older people. I was taught by Dra. Riera how to read the electrocardiograms; for instance, the indications of bradycardia and abnormalities of the waves pqrs. She asked us to have a picture of a normal heart rhythm on our ceiling on top of the bed so we could notice the abnormalities when testing patients. The rotation in the cardiology clinic was definitely something I would have not considered to be engaged in; however, for so many diseases; like in most pathophysiological studies, the abnormalities in the heart rhythms are associated to other diseases that I have realized are critical in a person’s overall health; most remarkably, the renal system. So I am now concerned about it. This last week I rotated in internal medicine… and many of them were previous chagas infections in which the parasites deposited themselves in the muscle of the heart; thus affecting its normal rhythm. I saw dialysis and kidney transplants. I also saw diabetics, with critical infections. It was painful even for me who was just watching…

La Argentina y sus gauchos

Mi viaje a la Argentina quedo con: Regresare, Argentina!! Llegue a Salta el vienes después de haber llegado del Salar el miércoles en la madrugada a las 4:30am. Sali a Salta el viernes a las 4am o sea que el jueves por la noche no dormí preparando mi maleta y lavando mi ropa. La Sra. Esther me llamo a un taxi para que me llevara a la vagoneta; lo bueno es que solo viajaban conmigo una chica de mi edad junto con su abuela. Fue tranquilo el camino con las dos a pesar de haber viajado por más de 8hras. En la frontera a la Argentina súper augusto porque el chofer hizo todo el papeleo por nosotras; ya acabando eso, nos pusieron pesticida todo alrededor del auto… ja ja ches argentinos. Llegamos a Salta y mi expectativa del lugar se supero, nada que ver con lo que me había figurado! La gente me lo describió similar a Tarija; pero la verdad, Tarija se queda un poquito atras. Yo me enamore de Salta y de su gente, así igual me imaginaba a los Argentinos unos mamones… nada que ver o sea hasta tuve pegue jaja mejor todavía porque los hombres guapísimos! Me quiero ir a vivir a Argentina, o sea eso ya está. Bueno, llegue a Salta y justo me dejaron en el centro, rustico y limpio a comparación de otras pequeñas ciudades de Bolivia. Lleno de boutiques de fashion por todas partes al igual que la bandera Argentina por todos los lugares jaja… yo creo que cuando dicen que los argentinos se creen, quieren decir que se creen argentinos... Llegue al hostal donde se quedaban los amigos ingleses, justo cuando estaban cocinando sesos de res… yak!… porque los probé y casi vomito. Después pre copeamos y listos para salir de noche a la Barcarce, la famosa calle de los bares. Que noche! Todo mundo nos hablaba y buenísima onda; los niños argentinos que guapotes… ;) Al día siguiente fui a las calles a visitar… ese mismo día en la noche llegando al hostal me encontré con la familia que es dueña de ahí; ellos viven en el hostal también… todos lindísimos y pues les gusta México entonces platicamos de todo con las chichas adolecentes acerca del mundo del espectáculo mexicano y así… después llego un señor a sentarse al comedor también, y ya cuando las niñas dejaron de preguntarme mil cosas y contarme ellas de Argentina, el señor se acerco y me saco platica. Bueno, resulta que este señor esta en un grupo musical y toca folklore argentino y chacareras tarijeñas pero el hizo en el pasado canciones mexicanas tal como la del pastor en folklore argentino, famoso nacionalmente y empezando el grupo musical ‘Los del Rio’. El y sus compañeros me invitaron a ir a escucharlos esa noche a la Barcarce otra vez. Esa noche solo fuimos una pareja de franceses que también se estaban quedando en el hostal y yo a verlos, mis amigos ingleses estaban cansados. Bueno el punto es que me cayo súper bien este señor porque esa noche me tocaba cocinar comida mexicana norteña ‘fajitas’ a los ingleses pero nadie me quería acompañar a comprar las cosas a la tienda y el señor vino conmigo, lindísimo. Y luego les invite a todos los músicos tacos de guacamole (por cierto, que difícil conseguir tortillas, aguacate, chile, frijoles porque los gauchos conocen todo eso con otros nombres). Esa noche en la Balcarce estuvo inolvidable, sus voces con el acompañamiento de la guitarra me llevaron a otro mundo. Aparte la sangría del lugar, la cacharpaya, estaba deliciosa! De ahí nos hicimos best friends forever. Al día siguiente, me fui con los ingleses a un café a ver el juego de Alemania vs. Inglaterra que lamentablemente perdieron feo… no se diga mi equipo después jaja. Saliendo del juego nos fuimos a un restaurant colonial argentino y pedimos parrillada, comimos de todo incluyendo sangre coagulada, riñón, intestino, panza y mas… de res. Y así a alistarnos para el juego Argentina vs. México. Regresamos a la cacharpaya para verlo en pantalla grande y obvio yo con mi playera que por cierto me prestó uno de mis amigos ingleses; claro, la gente no dejaba de verme. Los argentinos me veían y movían la cabeza como diciendo no no no puede ser. Después ya en los goles que lamentablemente nos metió Argentina, llegaban ellos a mí saltando no, gritando y me daban besos en la mejilla ja. Ese día en la noche, en la calle Balcarce, muchos argentinos que yo ni conocía me reconocían y me preguntaban ‘como va México?’ Uff, también esa noche en el centro había una presentación de orquesta sinfónica o sea así que caminando por las calles, la gente vestida con sus uniformes del AFA, y las banderas argentinas fue toda un experiencia. De ahí fuimos a un bar a celebrar con el resto de la populación pero llegamos hasta la 2am al bar después de haber pre copeado con los músicos en el hostal. Se paso el tiempo rapidísimo y mi transporte a Tarija salía a las 5am entonces me fui del bar corriendo sin despedirme bien de mis amigos ingleses. Todo bien de regreso, solo éramos yo y una chava de Tarija en la vagoneta; ella estudio en Salta y justo se graduó de la uni., nos hicimos amigas e intercambiamos teléfonos para después juntarnos y salir. En el camino a la casa de la familia Marques ya llegando de salta, me encontré caminando por la calle a los 2 australianos con quien estuve en el Salar y en Tupiza, justo ahí en Tarija… habían hecho la ruta del vino; esa noche nos juntamos para despedir. Hasta nunca mis amigos!

I went to a Chagas workshop the day after I came back from my salar-uyuni trip; very informative. We covered almost every aspect of the Chagas disease, from the discovery of it to the treatment; as well as the norms that are to be implanted. It took 5 hours to inform other health professionals that are to be capacitated to perform the Chagas treatment in their communities. Many of them came from rural areas of the department of Tarija to become capacitated to capacitate others in their districts. One of the main purposes of this workshop was to instruct health professionals that in order to prevent Chagas, one has to alert their communities about the disease, and the different treatments that are being performed depending on the areas, some are highly pandemic, some are being already controlled and others are not pandemic but the disease exists. It is important to be patient when informing communities the kind of treatment that is being taken because just like everything in nature, the parasite evolves to the drugs and/or people can become re-infected. The presence of the vector in a community has to be reduced to a 3% in order to proceed with the anti-parasite drugs; otherwise, the treatment is pointless since people can become infected again. To reduce the presence of the vector, pesticides are applied to these areas until the optimal percent of its presence is achieved. Then, one can start testing for the disease to begin any treatment.

Mi viaje al salar estuvo épico... excepto por el viernes-sábado que casi quería morir. Bueno, la cosa fue así, no?: El viernes a las seis treinta me llevaron mis host abuelos a la terminal… la sra. Maria ester me advirtió que en el camión robaban y por esa razón me puse buza la mayoría de la noche… me fui hasta el asiento de atrás para recostarme porque no había nadie… el camión viajaba toda las noche hasta las 6am llegaba a Potosí. Como hasta las 4am seguía despierta, con eso de que el camión para para subir más gente de por allí… puros quechuas con su lenguaje y con su olor que me mantenía despierta… después bajaban, y otros volvían a subir… Potosi es la ciudad más alta, a 4080km de altura! O sea que hacia un frio terrible… no sentía mis pies. Poco a poco subíamos las montañas, tipo el camión viajaba sobre rocas, tierra… hasta crees que había carretera… esas son palabras mayores! Tan siquiera viajaba de noche, porque así no veía lo angosto que era el camino por donde iba el camión con los 500m de altura de ahi al fondo de los cerros. El camión, no se diga… casi hecho pedazos. Bueno, eso no lo es todo, me quede dormida de tanto cuidar y estar con los ojos de pico, llevaba mi bolsa abrazada a mi… pues no sé como que me robaron mi cámara, mp3 y todo el efectivo que llevaba. Yo no me di cuenta hasta que baje del bus para comprar comida que ya no había ni un billete!! Grrrrrr. Después de no dormir bien toda la noche, pasar frio y aguantar a esos quechuas, me asaltan?! Llegando a potosí, no sabía qué hacer, o sea mi cámara! Como iba a tomar fotos del salar? Eran las siete que tome el camión al centro de la ciudad… con un cansancio por la altura de la ciudad. Potosí está casi lleno de gente no habla español. La mayoría de la población es indígena. Quien sabe que decían unos que me seguían… entre rápido al mercado que ya estaban empezando a abrir y me senté a desayunar café con pan, le conté a la doña lo que me había pasado porque me pregunto porque me veía tan triste y ella me dijo que en potosí está lleno de lagartijas. Uy, yo que quería ir a las minas de plata de potosí. Me fui a buscar el camión que saliera a Uyuni lo más pronto. Y entonces me escape de Potosi… El camión a Uyuni salió a las 11 y media y llegamos a las 6pm, hacia un frio espeluznante. Me vi obligada a comprar una cobija. Me quede en un hostal esta noche porque los tours al salar salen a las 10am. Mientras yo, como loca buscando cámaras digitales. En Uyuni no hay nadaaa!! Encontré una cámara bien entonces la sra. me la abrió para ya dármela cargada con memoria y todo, cuando veo que no me alcanza… voy corriendo al único cajero y, what! Fuera de servicio. Ya había usado casi lo último en el tour de dos días del salar. Tenía 10dls pero tenían una roturita y nadie me lo aceptaba… solo me faltaban 70bolivianos para pagarle a la sra. pero nadie me cambiaba. Hay mucho turista en Uyuni pero cada quien en su rollo. Le dije a la sra. que mañana, no sabía cómo pero iba a conseguir ese dinero. Bueno para no hacerla tan larga, en la mañana me fue tocar la sra. de los tours a mi cuarto, y me dice que mi tour se cancelo y que solo había de 1 día. Obvio el de un día era más barato, y fua! My Money back :) El poder de la intencion! Y ya fui y le page a la de la cámara. Fue ahí donde se acabo lo peor de mi viaje. Lo mejor estaba por venir…!! Me dijo la sra. de los tours que en el de 1 día iban 6 chavos ingleses y australianos, y que estaban guapos todos… jaja. Nuestro viaje empezó, y todos increíblemente amables como para decir que hasta nos hicimos súper amigos todos. Y de ahí ya no nos separamos hasta que yo me regrese a Tarija. Todos fans del mundial, o sea que su viaje en Sudamérica se planea conforme estén en un lugar para ver un juego. Bueno, regresando al tema de mi inaudito viaje, no hay palabras para describir el paisaje del salar! Wow. Las fotos dicen poco de hecho.

Bueno, esa noche que llegamos a Uyuni después de estar en el salar, cenamos juntos y de ahí tomamos un tren a Tupiza a las 2am; o sea que mientras esperábamos en la terminal jugamos cartas, pato pato ganzo, a las escondidas para mantener calor en el cuerpo. A los nuevos amigos y a mí, nos toco estar sentados al lado sin planearlo. El tren estuvo de lujo después de los camiones, obvio… pedí asiento ejecutivo para evitar más pérdidas, hasta lonche y mate nos dieron. Llegamos a Tupiza; mi plan era cabalgar e irme esa misma noche a Tarija pero los chicos me convencieron en quedarme para hacer el paseo a caballo el día siguiente todos juntos. Entonces pase la noche en Tupiza, y pues ya conocí el pueblo; parecido a Tarija pero en una versión pequeña y feíta. En el hostal de los niños había tele con cable y pues vimos unos juegos, y compraron pelis… vino baratito e hicimos pijamada. El paseo a caballo fue lo mejor, amo el cabalgue…arre arre! Todavía ando adolorida. Creo que este fin nos encontraremos en Salta, Argentina. Aunque estuvo larga la historia me salte más de la mitad!

Hoy en la noche, se supone es la noche más fría en Tarija entonces la gente celebra y todos comen hotdogs con ponche de vino en fogatada pero con eso del calentamiento global, mama naturaleza no enfrió tanto. Aun así, celebramos y platicamos con los amigos de mis host.
Bueno, mañana voy a asistir a un curso de Chagas!! El tiempo corre…

De mochilazo

Me voy a Uyuni. Voy a visitar el salar este fin con eso de que hay feriado el Lunes. Hoy tomo el camion; sale a las 6:30pm. Llego a la ciudad de Potosi a las 6am... voy a turistear en este famoso pueblo, de alli salgo a uyuni y de uyuni tomo un tour al salar... duran aprox. 2 dias. De regreso me vengo por Tupiza y de Tupiza a Tarija. Planeo llegar ya sea el martes por la manana o noche... depende. No me llevo mi compu asi que no voy a estar muy comunicada pero traigo mi cel para marcarle a la doctora por si algo, e intentare meterme a algun cafe para avisarle a mi Papas que estoy bien :)

Chagas Treatment

On Monday the 14th, I began the Chagas Prevention Project. I went to the fields to visit kids from 1st grade to 9th grade in public schools to the follow up of the Chagas treatment. What they do is, they visit every single school in the Tarija department; including those hidden in the mountains where the poorest people live. They take a blood sample of every single kid. They examine the blood and if the test comes positive of Chagas, they send letters to the parents notifying that the kid will have to be treated for a 2-month period with an anti-parasite. The drug is strong; therefore, parents have to make sure the kid eats well and does not get allergic reactions. The daily dose of treatment varies on kid’s weight and age. It is taken twice a day after a full meal. Every week, the doctor pays a visit to the infected kids to check on treatment and adverse reactions. Also, she randomly visits households to make sure kids aren’t lying about their intake… but , what is it so important, right? Chagas is a fatal infection if left untreated; which can be seen in adults that were not treated early. For these people all that is left is to treat the symptoms. Now, what is Chagas exactly? Chagas is a disease produced by a parasite called Tripanosoma cruzi; which primarily affects the heart and the intestines, leading to an irreversible damage. Chagas is transmitted through the vinchuca. The vinchucha bites people to feed itself. When it bites, it leaves its feces on the skin in which parasites are found. Then when people scratch the bite, parasites enter the organism. The treatment is carried out with a drug called Benzinidazol for 60 days with weekly controls performed by health professionals. The treatment is effective when Chagas is detected and treated on time. There is hindrancein this process; people that we visit are extremely poor and uneducated. We extremely encourage parents to manage their kids treatment; however, most of the time they don’t speak Spanish, they don’t understand the seriousness of the illness, nor they know how to read/check the drug schedule that is given to them. Kids and parents from the fields are too shy and get intimidated by strangers that sometimes this limits their capacity to pay attention to what is being explained to them. Fortunately, these visits are a weekly basis. Once they become familiarized with their doctor, they are willing to listen. These people may not be educated but they are extremely welcoming and charming. They barely have food, yet they offered us something to eat with a big smile on their face. They kiss those who help them, and say a million times ‘gracias’. We visited communities Monday, Tuesday, and today; and I sincerely say that I certainly experienced pure Love.

Vino y no tomo?

"Si vino a Tarija y no tomo vino, vos a que vino? Regresese por donde vino." -Dicho chapaco

El sabado viaje a las famosas y únicas viñas bolivianas; aprendí mucho de la historia de Tarija y deguste de sus famosos vinos, quesos, jamones y dulces. La combinación de un vino de mesa con el queso fue una delicia. Bolivia tiene solamente un estado que produce vino y este es Tarija. Se diferencia de los vinos chilenos y argentinos en que la uva crece a una altura de 1400m; lo cual es raro. Pero Tarija ha logrado producir diferentes tipos de uvas a pesar de la altura. Me llamó mucho la atención en los viñedos que justo enfrente de las hileras de la planta de uva se encontraban rosales. Estos me imagino se han de ver hermosos en el verano. Son plantados para prevenir la plaga en los viñeros. Cuando las rosas tienen su zona centro, en donde se encuentra lo más dulce de la flor abierto y tipo masticado, es porque hay plaga; es entonces cuando se aplican los pesticidas sobre la planta de uva para eliminar la plaga y ‘curarla’. No acababan los natos de decirme que hubiera mejor venido en febrero, cuando todo está colorido y dando fruto. Pero a pesar de que no había ni uvas y ni rosales, a mi me encanto. Yo, felicísima (como dirían los chapacos) con poco.
Les contaba a mi familia chapaqueña que casi todos los que me encontraba en el camino me preguntaban que de donde era, y obvio que una vez mencionado México todos me preguntaban algo de ahí. Lo que más les sorprendía era que una mexicana estuviera en sus tierras. Me preguntan la mayoría, Y porque vos has venido a Tarija? Ya pensé bien mi respuesta después de tanto porque la mayoría de las veces no tienes razón especifica del por qué haces las cosas. Así que yo les contesto: porque busco vida pueblerina rica en cultura en un país no muy acreditado mundialmente justamente en Sudamérica. Y de hecho no pude haber elegido ciudad más precisa! La gente de aquí, ama ser de aquí, sabe todo de aquí, y no acaban de alabar a Tarija en ningún momento… o sea exageran. Pero si no fuese por su entusiasmo de ellos, no me súper encantaría Tarija como lo hago...jajaja... Bueno después de tanto, mi familia ha decidió nombrarme embazadora mexicana porque igual yo les cuento a todos qué hay con México y así. Unos chavos en la Casa Vieja del valle de la Concepción me dijeron ‘shakira’ jaja y ahora la familia me dice shakira embazadora mexicana (uno puede ser distinto que no? jeje) pero son súper amables todos <3
También tuvimos noche familiar en la casa de campo de la familia de Karla el sabado en la noche hasta las 2am con fogata y todo. Una guitarreada con ponche, adivina que llevaba el ponche? Ya, vino! Fue una experiencia inexplicable. Para decir poco, me divertí muchoo… son gente de mente super abierta, con chispa y les encanta México y la comida mexicana. El esposo de una pariente también era de México, el es de Aguascalientes y profesor del ITESM de merca en campus Zacatecas. Ahora enseña desde hace 8 meses en una universidad privada aquí en Tarija. Yo y el nos la pasamos riéndonos de lo que estos locos Bolivianos piensan de México y sus tradiciones. Y luego fue viceversa y después cambiamos lugares.
El domingo celebraron en cumple de Don Javier Marques, el abuelo de la casa donde me hospedo; uy y vino de familia… todos comimos y platicamos.
El viernes comencé mis clases de Portugués! Agora, eu seu tudo um coração ;)

Today, I went into the Quirofano again. I just realized that I’ve been following a surgeon who unlike other surgeons here; he mostly performs surgery. Other doctors do consultation and/or urgent care about half the time they do surgery. But I was assigned Dr. Martinez because I told CFHI in my application that I wanted to become a surgeon. Ha, love it. I saw another removal of the gall bladder today … what was most surprising about it all was the fact that the patient was super young! I would say about 25years of age; worst, she had 8 rocks of precipitated bile! It can be seen in the pictures (scroll down). The scar that was left from removing the whole gall bladder was so small… aesthetics! The patient ordered. Obviously, she’s young and got a full life. Then, after surgery, I lost my surgeon in the hospital; so I decided to go to urgent care to take a look at ‘fresh’ injuries. I have not gone to the hospital in two days since I had some stomach discomfort after eating from the street but I felt good today; I hope I just keep feeling better. I arrived to the clinic and there were two nurses waiting along with a patient lying on the bed. He had fallen and hit his frontal jaw; there was a small aperture under his mouth. The nurses looked at me and asked me if I could sew the man (really?!), I said: sorry, I am not allowed (nor do I know how to apply anesthesia)… we waited like 10min and no doctors came. They were probably watching El mundial (had to be Latin America). The nurse told me: I am not allowed to sew the man either, but I will do it since no one’s coming. She applied the anesthesia and sew the man; then someone knocked at the door and they covered the man… he was awake, and listened to everything but he didn’t care. They gave him a prescription of tetanus and a 500mg of antibiotics every 8hrs. He was released. The hospital was a little empty, and from the far, I heard some GOOOOL noise, so I followed it. South Africa had scored a gol on Mexico. The patients, some doctors, residents and I watched the game. I’m glad they wanted Mexico to win, probably just because I was mad yelling there haha. Ah… but they won’t miss the game tomorrow; Argentina is playing (awhhh cute).

Hoy no asistí al trabajo porque no me sentí bien en la mañana. Es por eso que tuve tiempo para rellenar mi blog. Creo yo que es una infección intestinal pero me conto la familia que puede ser un resfriado. Normalmente les sube la temperatura corporal y les duelen los huesos junto con un dolor en el estomago; lo cual es parte de la incubación del resfriado. Puede ser pero me he sentido con nauseas también. La doctora Karina viene en un rato a revisarme, espero no sea nada que me evite ir al hospital (a trabajar, obvio), a viajar y a probar comida. Grrrr.

Today was mouth-opening. I went to the hospital with a positive attitude, ready to learn and help; you heard me yesterday. Dr. Martinez took 3 med students and I to a conference room where each discussed a case. They had to read their data and medical history, physical state and complications; then they had to come up with ways to improve the state of patients using surgery as the last resource. There was a case of patient with gall stone symptoms and a wound infection from a previous operation a partial gall bladder removal. It was concluded that the patient still had gall stones in the leftover of gall bladder; thus, the symptoms; and the infection of the wound through where the caterer was inserted occurred due to negligence. Another case was a male patient with a hernia in left lower abdominal part; the doctor discussed the different types of hernias mostly probable for this case. What to do with this patient was left pending once the intern became familiarized with hernias. The last case had to do with a young woman who had a mild to severe pain in the upper part of the abdomen. She was taken to emergency on Friday; she was given pain medicine and allowed to go home. Then, 6hrs later her pain was severe and was interned. Her case was considered grave after realizing that she had had a stomach perforation long ago, the perforation went through the pleural cavity and caused inflammation due to excess fluid in the cavity. The internal wound was infected, and it had now penetrated the lung pleural space. The med student reported breathing problems which at the moment seemed to be explained in the diagnosis. Her platelet number was low and she was weak, pale, and showed jaundice as well. Liver had shown damage. I had to be present when the doctor told the family of this girl that she was in a severe state and had to be taken to intensive care. However, I followed the doctor personally to the intensive care center and there were no available beds. The family was told this. It was hard! Even though I see how bad this case is, in my perspective, the doctor was direct but the family did not fully take it as serious as it actually is. The infection has gone through most of her body and organs! And there is no bed for her in intensive care; I pray she gets the attention she needs even though it’s somewhat late. On the other hand, that was not everything today. A35 year old man died. I saw him after 4min of being dead; with his eyes opened staring at nowhere, with a stiff body, his lungs seemed to be holding the air still because his chest was pumped up. He passed away from appendicitis; I believe there was some colon problem as well. The doctor told me that he had been diagnosed too late. Dr. Martinez seem worried for like 3hrs about this case; he told me that he was mostly worried about the family because they had come this morning to see the patient and they were denied the entrance. The family yelled and got violent at one of the interns saying what a bad job they were doing. Two hours after they left, he was announced dead. After this harrowing moment, I felt like I wanted to faint for a second; then I felt nauseous for the rest of the day. I went to the clinic of urgent care after taking air with Dr. Martinez; the things I saw were so bloody such as dog bites, anesthesia injections to wounds, burns, knits to little kids, diabetic feet, and removal of toe nails. All these were painful to the patients; however, I don’t thing I could’ve seen anything worst today. Then I was released from med hell.

First Rotation

After arrriving to Tarija yesterday with little time to get to know the city, I was told I started to rotate on Monday so there I was this morning with my scrub, coat and tag. I followed Dr. Humberto Martinez, my respect to this man. He is busy. I arrived at around 8:30 to the hospital San Juan de Dios, to my surprise it was already crowed out side and not to mention the consultation list! We did neurology and follow up of patients that were performed surgery; along with me, there were other medical students from Tarija. We were done with follow up; and Dr. Maritinez and I headed to consultation. To my surprise and disadvantage, I found out that medical doctors have to do a lot of reading and writing and signing and checking and writing, and writing and more writing... (in the U.S. probably, typing?). They need to check history of patient and fill out worksheets.
A really interesting thing I found out today was that in Tarija, and only Tarija has a special insurance to all its residents where there is no discrimination to farmers, to homeless, to the middle class, to the rich, to the poor and to those who don't go to school or don't work. The insurance covers mostly everything! How amazing is that? There is free health care to everyone :) My dream world! Ah Love is not that scarce after all... what a relief.
Well, the most exiting and unforgettable moment of today was me going to El quirofano and watching live an operation of the gall bladder removal due to stones. To my understanding, here in Tarija; it is the most common operation performed. This is because of the excess of fat and cholesterol that is added to the diet of Tarijiences... they like to eat meat and chicken and pork a lot! and they also consume them with skin which stores much fat in it, then they fry them or add extra oil with bread and lots, lots of cheese! Yes, did we learn something today?
I removed all my clothes before surgery and changed to cleaned scurbs, a hat, shoe covers, and a mouth cover. I stood and watch the full operation for 1 and a half hours. Not gross, super interesting! The gall bladder was removed completely and the gall stones were so visible.
During consultation, most people came complaining they had a pain posterior between both the ribs; others came with the radiographies and there they were, tiny gall stones. For those, Dr. Martinez said: "we need to operate". A woman was scared when she heard this, and she wanted medicine; however, it was too late for drugs. One of my favorite part of all day today was the way patients looked at me. Yes, they looked at me in they eye and told me what they were feeling as if I was a professional. I loved they way they trusted me in telling me in detail how they felt. The doctor was busy reading their history so he wouldn't look at them thus they turned to me to tell me... ahhh, I can't believe this is real life. I want to gain more knowledge to tell them what to do to get better. To me, it is an ease to see people worrying about the physical pain they feel because this means they care about themselves; and for a foolish reason, I just love to know that.
What a day and it is just the first one, and only 3pm. OMG. I can't wait for more.

El viaje estuvo pesado, de eso seguro!! Desde que asi ayer me senti algo mareada y con falta de oxigeno depues de tanto sube y baja de avion, y con la altura de La Paz de mas 5000m sobre el mar... ya no se si me emociona regresar jaja si si, igual y me quedo..ntc
De phoenix a chicago de chicago a miami de miami a la paz, "descanso" en la paz y al dia siguiente, de la paz a tarija. Porque puse entre comillas descanso...?.. La verdad no pense que fuera a salir tan pronto de fiesta. Con eso de que hablo espanol y me lleve bien con el nino de 17 anos con quien vive la senora que me dio hospedaje en la paz; ah porque para esto, es fan de Mexico jaja que por cierto en Bolivia les encanta Mexico y mi acento blahhhhhhh... Me dijo "ah vos hablas como en las novelas y vos te llamas Andrea tambien" jaja.. Bueno bueno regresando al tema, fuimos a un antro fresa de la paz.. Forum.. y estuvo 'churo'. Saliendo comimos anticuchos que venden en la esquina.. es corazon de res guisado acompanado con una mini papa pelada y picante de mani.. la salsa fue lo mejor! Claro, en la tarde tomamos un te de coca, y te explico que sabe a .... Te! jaja nada super especial. Tanto que probar y tanto que ver y tanto que aprender... ya fui a conocer el hospital en el que voy a trabajar... es publico, y da un poco de miedo. Es algo antiguo y tiene unos posters de muertos, osea ni idea porque. Esta obscuro, uy! jeje. Tienen todo, sala de medicina interna, sala de cirujia (eaaa!), ginecologia, cardiologia, neurologia, maternidad, sala del quemado (jeje), etc.. ya quiero traer mi bata oh siiiiiiiiiii.
Luego cuento mas, por ahora chao que ya tengo sueno..
